Faculty Associates and Substitute Teachers
All Faculty Associate and Substitute Teachers are hired through our applicant tracking system, ISU Jobs.
Faculty Associate
The following documents will need completed and/or submitted to Human Resources in order to hire a Faculty Associate:
- Position and Funding Request for the Budget Office. Please refer to this user guide if you need assistance completing the form.
- Offer Letter
- Personal Data Sheet

Form I-9 - Employment Eligibility Verification
- All employees must have a valid I-9 Employment Eligibility form on file with the University. These forms are completed electronically.
- All international employees must complete their I-9 form in Human Resources in the Nelson Smith Building Room 101.
- The employee will need to present valid work authorization documents when completing their I-9. All documents must be originals; copies are not considered to be valid documentation.
- Each hiring department should have at least one staff member who has received I-9 training. Contact Human Resources at (309) 438-8311 with any questions regarding the I-9 form.
- Affirmative Action Search Paperwork
- Official Transcripts showing highest degree conferred
- A contingency will be put on the contract if not received
- Illinois Teaching Certificate
- No expired or out-of-state certifications accepted

Payroll Forms
- Illinois W-4, Federal W-4, and Direct Deposit Authorization
- SSA-1945 - Statement Concerning Your Employment in a Job Not Covered by Social Security
- Fingerprint Form and Fingerprint Memo
- TB Test Results
- Must be completed no earlier than 90 days before start date
- Physical Exam Test Results
- Must be completed no earlier than 90 days before begin date
- CANTS 22 DCFS Mandated Reporter Form
- Selection Form through ISU Jobs
Coach Faculty Associate
The following documents will need to be completed an/or submitted to Human Resources in order to hire a non-continuous Coach Faculty Associate:
- Position and Funding Request for the Budget Office. Please refer to this user guide if you need assistance completing the form.
