Forms

As part of the Reasonable Accommodation Procedure, employees and applicants will need to complete two forms:

• Employment Accommodation Request
• Medical Information Request

Any medical documentation submitted to the Office of Human Resources is confidential and will be filed separately from applicant records or employee personnel files.
