Injury Log
Supervisors, Forepersons, or other appointed and authorized individuals are required to file a workplace injury report on the Injury Log at the Environmental Health and Safety website. The Injury Log provides all necessary forms as well as contact information for the University Preferred Provider for medical treatment of work-related injuries.
