If you were previously a student at Illinois State University or the laboratory schools, you will need to change your name through the Registrar Service Center.
If you were not previously a student at Illinois State University or the laboratory schools, you will need to change your name through Human Resources.
To complete the name change, you must provide a copy of your marriage license, a court order, or a dissolution decree certifying the name change. Send a photocopy of one of these documents, a copy of a photo ID with your new name, a copy of your new Social Security card, and a written or faxed request that includes your University ID (UID). Be sure to sign this request, as your name change cannot be processed without your signature.
You may also elect to bring the appropriate documentation and request into the Human Resources office. This is preferred if you have insurance benefits with the University.

Faculty and staff can set up preferred names through ULID Account Management.
It can take up to five business days to process a preferred name change.
Students can set up preferred names through the Registrar Service Center in Moulton 107. Students must make this change in person with a valid photo ID.
Once you have completed the process to change your name, you will need to complete a ULID Change Request using ULID Account Management on the Help Desk website.
